After dropping at patient at hospital you return to the ambulance to restock the bags. As you reach into the draw to restock the cannulas, you feel a sharp puncture to your index finger. On a closer inspection you note that this puncture was caused by a needle, that had been used and was not disposed of correctly. You must complete the incident report based on this injury.

While restocking the cannulas in the ambulance, I reached into the drawer and inadvertently felt a sharp puncture on my index finger. Upon closer inspection, I discovered that the puncture was caused by a used needle that had not been disposed of correctly. The needle was contaminated and caused an injury to my finger.

Identify Outcomes of Analysis for Action

The injury resulted in a minor puncture wound to the index finger. It did not lead to any severe or lasting harm and hence was negligible. Given the circumstances described, it was highly likely that such an incident would occur if a used needle was not disposed of correctly. While the impact of the incident was negligible, the high probability of such an event occurring in a similar situation indicates a moderate level of risk. This suggests that corrective actions should be taken to prevent future incidents of this nature. Therefore it must be ensured that clear and effective protocols are in place for the safe disposal of sharps to minimize the risk of similar incidents. Routine checks must be conducted in the ambulance to ensure that all sharps are properly disposed of, and that all safety measures are being followed diligently. It must also be Ensured that the ambulance is well-lit, clutter-free, and equipped with necessary safety equipment to minimize the risk of similar incidents in the future. Prompt reporting of any incidents, no matter how minor, must also be done to ensure that appropriate actions can be taken in a timely manner.
