Introduction to Bernie Gerard

Bernie Gerard is an RN at the University of Medicine and Dentistry in New Jersey. He has been assigned to the unit of major trauma and surgical intensive care unit. He is in the third month of his orientation but on a busy Saturday, he was assigned to the coronary care unit by the nurse supervisor. To the new assignment came a dissatisfaction within him as he was starting as an oriented, he felt he was not in quick judgment will be able to handle the patients at the coronary care unit as he deemed he was unable to task under risk and he did not know the equipment of the unit at his disposal. So he is cornered by the nurse director, nurse supervisor, and the nurse at his care unit.

The journal and Bernie's daily vibration of his life is personally jotted and its quite structured narrative and shows all the detailed vitalities attached to the case briefing.

Challenges and Strategies That the Nurse Experienced

The challenge that the nurse experienced was that he was shifted into another critical coronary care unit from major trauma and surgical care unit in his third month of orientation. He also has to work in that unit but he is unaware of how to handle the patients at that unit and he does not know about the equipment and medications that are available at the unit. So Bernie makes sure he is heard and when the nurse supervisor takes him to the nurse director he can convey the message about his inexperience to handle the situation and coronary care patients. He also informs the nurse supervisor that he did not know the protocols of the binary care unit and he is also unaware of doctor contact procedures. He was also unaware of his safety as time is the functional dynamics of any nursing protocols. He also informs how his inexperience can jeopardize the patient's care and safety. He mentions his novice experience in unfamiliar settings.

So these changes in the care unit without any experience were the challenge faced by Bernie, the nurse.

The strategy of NSB104 that Bernie used was proper and clear conscience communication with the nurse director. He was effectively clear in points. So the nurse director listened to Bernie's point and arranged an orientation for him to work with novice experience. According to the protocols of the workshop and Clinical nurse leadership program in the sections has notified orientation programs for the new resident nurses as they are certain procedures to work in any of the critical or intensive care units. The New resident nurses should know and have all the knowledge about the whereabouts of the unit, the environments of the unit like the doctor contact procedure, register of medications and equipment and know all the principle working functions of intensive critical electronic such as ventilators in pulmonary care units or application of intravenous delivery systems (Weiss, Tappen & Grimley, 2019).

Each intensive unit has its unique machines that operate to look after the personalized requirements of the patients. So according to the protocol set up, the nurse director was able to arrange for the orientation to 3month old experienced nurse Bernie to work in the coronary care unit. Other things that are mentioned in the protocol is the safety of nurses in the working environment (Brunkert et al., 2019). The older nursing has to be responsible for the newcomers as they will train and act according to what are the needs of the patients so that the safety of the patient party is not hampered (Miles & Scott, 2019).

Everyone i.e the patients and staff safety is crucial to health care services. As the intensive machines are extremely sophisticated and priceless so it is a must for the trainees and staff to know about its operating principle and working principles (Cimiotti et al., 2019).The clinical nursing leadership guidelines take care and direct the nurse group to function according to the protocols and be responsible for the actions that they perform while in the facility and care units. Skilled communication is pivotal in the nursing province (Doherty et al., 2019).

Application to The Clinical Case

After going through the challenge and strategy used by Bernie in the story as a student nurse I would live to grow the protocols of clinical nursing guidelines to look into the pros and cons that are concerned with my work profession, considering all the aims and objectives and look into the disciplinary guidelines to know about the regulations that are to be maintained by a newcomer who is going to shift and look after patients through his life. As a nursing student, I would use the communicative skill and confidence to let out your voice to the governing authority to inform them about the shortcomings of a newcomer that is to be faced while changing from kind of patient ward to another ward. As a novice practitioner, certain norms are to be dictated.

Profuse and direction of orientations should be one of the early practicing habits of a newcomer nurse. Effectively 7days of proper orientation and direction are required to perform under the ecosystem of the units (Arnold and Boggs et al., 2019) There are strategies for compliance to remain firm and affordable in dealing with the situations of patient safety. If Bernie would remain silent in the first place, then start working in the coronary care unit without supervision and help he could endanger the patient's life and it could give a sense of insecurity could always work in him, so he could be unable to function under any working profession as a team member or individual responsive unit. The confidence in the workplace offers determination and more focus in the work and helps out each other.

Proper coherence and clarity in communication should be the environment of this kind of work hierarchy and task performance (Sakr, Metwaly & Taha, 2019). The strategy of Bernie was wise and his way of deliverance of his working inexperience and his thought of safety from his conscience has nicely shaped his future while after working for four years he thanked the affords of nurses union to have work orientation in the nursing system of his University. His thought process and the act of quick wisdom saved his career and became a schedule of work in his University. With the changing dynamics of the medical profession, novice practitioners are at an advantage.

The changing politics of nursing leaderships have looked forward in defining their policies and strategies that will formulate the working principles of the nurses from their starting periods as an intern or the most nursing senior staff. A hierarchy in regards to professional seniority is a pivotal unit (Acevedo-Neuvo et al., 2020). It also provides information about the duties and responsibilities of nurse supervisors and nurse heads of each personal care unit. So that their functions and disciples which should contain the section were assisting and working support to newcomers interns of nursing staff (Acevedo-Nuevo et al., 2020). Bernie's approach in dealing with the situation shows his prolific decision making and quick analysis of the working protocols.

But he puts his referendum and he does not want to risk the health of the patient. He estimates that his novice experience of 3months and a major shift from one unit to another without the proper knowledge could comprehend the total functioning of work as his one mistake and his unawareness of the environment could risk everything (Brunkret et al., 2019). Suppose there are many cases where a newcomer can perform the task of ventilation in a pulmonary care unit as they are inexperienced but the proper explanation of working experience could have helped.

Suppose in a dialysis unit, a new nurse cannot assist in the first day because of the change in the blood delivery system could result in reverse functioning of the blood dialysis chamber, in particular, jeopardizing the safety of the patient, as was the case in one the good university of Britain. So there are more instances where inexperience skills of new nurses or student nurses can hamper the patient's health or aftercare surgery. The rehabilitation process and approaches taken are different for different clinical situations. From a minor orthopedic ward to critical brain surgery units (Duffield et al., 2019).

Reflection of Only When It’s Safe by Bernie Gerard

Two key insights that I have learned from the story:

It is quintessential for all the newly joint resident nurses to learn about all the whereabouts of all the clinical procedures of every ward by undertaking routine shifts of orientation during their period of internship while working in health care centers. As a responsible nurse, I would convey all the detailed history of my learnings so that I do not jeopardize the safety protocols of the patient. For a patient, it is the care that brings them to the hospital. Their medications and timely record of their routine to provide the patients personal pharmacology and his requirements so that their safety is hampered and our career has a kickstart as a regular medical practitioner. The environments of the clinical nursing leadership point regarding the orientation period of the nurses and then joining as the caretaker of the ward or critical units such as the coronary intensive unit or CCU (Brown & Ashcraft, 2019).

Another skill that I learned from Bernie is to have proper communication skills to let the nurse director know about the problems to shift from one ward to another specialized ward. It is very important to speak and talk to the seniors with proper discipline because a nurse cannot refuse to work as the protocol of a nurse senior in command.

Communicative skills, comprehensive analysis of the nurse director, and Bernie's self-awareness particularly confused the working function of the story. The effort taken by Bernie has been used by the university to include in the program of nursing management. The entire setups and network in the management had new protocols to deal with new nurses and staff. Time and working experience with the profession helps to determine the ever-changing dynamics of nursing leadership and management (Salamonson et al., 2019). These mottoes must be included in all the nursing protocols so that it can render support to student nurses and help them come with newer ideas as the novice practitioners must assess and evaluate the nursing protocol such as orientation and efficacy.
