It has been reported that play has a significant role in the development of a child (Whitebread et al., 2017). The physical plays have a relation with the motor development of a child and help the child in developing skills of effective social interactions. The block games played by children lead to spatial processing and mental rotation among children (Whitebread et al., 2017). The games involved wordings played a significant role in language development among children. According to Sawyer (2017), the children who consider and perceive activities as a play are more focused, motivated and attentive in nature. These kinds of children show better signs of achievements while performing any task. The games played by children impart better-thinking ability and critical reasoning to them (Sawyer, 2017). 

4. Ella requires hospitalization for a fractured femur. Discuss the impact of hospitalisation on her and the family.

The impact of hospitalization can be stressful and anxiety provoking for Ella. According to Rokach (2016), can lead to the problems of stress and feeling of loss of control among children. The major reasons behind the stress of children due to hospitalization can be loss of control, separation, pain and body injury (Rokach, 2016). The children can experience anaclitic depression due to hospitalization which is categorized into three different phases namely phase of despair, phase of protest and phase of detachment (Neumann & Inwood, 2016). This can impact the other family members including parents and siblings of Ella as well. The feeling of loss can be depressing for other siblings and the prolonged hospital stay of the child can lead to anxiety, anger, fear and burnouts among parents of child (Kuman & Avabratha, 2015)

5. Jason and Sophie are infertile so have adopted Hamish. Describe the impact of infertility.

According to Anokye, Acheampong, Mprah, Ope & Barivure (2017), infertility is associated with a range of emotional, financial and emotional and socio-cultural problems for the couples. It has been reported that infertility can lead to physical and verbal abuse, stigma, exclusion and divorce (Anokye et al., 2017). Increased level of despondency can occur among those couples who are infertile.  Hasanpoor-Azghdy, Simbar & Vedadhir (2014) stated that infertility can result in emotional and psychosocial problems among females which can impact the well being and mental health of these females. The impacts of infertility can be devastating in terms of low self-esteem, turmoil and failure feelings for the couples.

6. Identify the principles of a health assessment.

1. The process of health assessment should comprise of a collection of data, proper documentation and evaluation of the health of the client and response to the required interventions (NCHK, n.d.). 

2. The assessment should be done regularly and on time.

3. A comprehensive assessment comprises of the psychosocial, physiological, cultural, spiritual and environmental health status of the client (NCHK, n.d.).

4. The documents of health assessment must be accurate, objective, concise, clear, recent and specific in nature.

5. The health assessment should be done by interaction of clients and nurse in healthcare settings.

6. The information that has been collected during health assessment should be communicated to other health professionals. The confidentiality of the clients should be maintained during health assessment.

What are some potential solutions for the problems?

Barry is male of 59 years old who has married to Michael who is also a male. This has represented that he has a different sexual orientation than general population.  According to Lee, Oliffe, Kelly and Ferlatte (2017), the gay man is a subgroup among which suicidality and depression are more prevalent.  Also, these men are more vulnerable to certain sexual health issues like HIV (Lee et al., 2017). Thus, Barry can encounter the complication of depression, suicidal thoughts and sexually transmitted diseases.  The other issues can be substance abuse like tobacco, smoking, alcohol or many others (Lim, Brown, & Kim, 2014). It has been stated that the education and counseling of the patient can help in relieving him from depression and suicidal thoughts (NIH, 2017). The medical assessment and treatment help without discrimination of people can help in treating the complication of HIV (Lim et al., 2014).

b) When conducting a health assessment on Jake, what are some problems you may encounter?   

What are some potential solutions for the problems?

The common healthcare problems that can be encountered by Jake are pneumonia, cough, cold, vomiting, and fever (MedlinePlus, 2016). In addition to it, skin problems like cradle cap of diaper rashes can occur. The gentle cleaning of the diaper area and use of creams can help in treating diaper rashes (AAD, 2018). The proper medication of the child can help in treating cough, pneumonia and other complications (MedlinePlus, 2016).

8. Sammy resides in supported accommodation. Summarise the 4 Core Standards for Practitioners 

who support people with a disability.

 The core standards for the practitioner who are engaged in supporting people with disability are as follows:

1. Person-centred healthcare assessments and healthcare plans development

2. To provide behavioral and communication support (NSW Government, 2014).

3. To provide coaching and teaching support to those who have complex and chronic healthcare needs

4. Mealtime management which includes management of dysphagia, enteral nutrition and provision of nutrition for wellbeing and health. 

Other principles are as follows:

5. It is the legal obligation for the practitioner to promote the rights of people for self-determination and expression in decision making (NSW Government, 2014).

6. The practitioners must work for the participation and inclusion of the disabled people in society.

7. The services provided by the practitioners must be assessed, plan, deliver and evaluate according to the strengths of each individual (NSW Government, 2014).

8. Regular feedback needs to be taken by the practitioners from the people with the disability for their reviews about the quality of services provided and their satisfaction.

9. There should be effective management of the services and practitioner must be accountable for their actions performed in order to maximize the health outcomes of the patients

The aim of the NSW Family and Community Services Person Centred Health Care Assessments and the Development of Health Care Plans Core Standard for Practitioners is to develop a framework for regulating the practices of healthcare professionals working in the specialized field of community health services(NSW Government, 2014). The standards that have been provided in the policy have been structured to guide the nursing professionals for the provision of safe and quality care to the clients (NSW Government, 2014).

b) What is the aim of the NSW Family and Community Services Mealtime Management for Nurses Practice Package?

The aim of NSW Family and Community Services Mealtime Management for Nurses Practice Package is to support the practitioners who are providing services to the people with disability so that these nurses can provide efficient and consistent services to the people. It provides a framework for good practice for the management of mealtime in order to ensure the safety and quality of services provided to the people with disability (NSW Government, 2014).
