Designing Customer Profile Form of Pine Valley Furniture
Customer Profile Form is used to collect data from customers so that Pine Valley Furniture can create and maintain their customer database. To encourage new users in filling the profile form it has announced 10 discount on total order. In the same way existing users will have this discount if they fill the form.

Company name, system generated Customer Id and date is already present on the form.
Other than the text boxes for data input, radio buttons are used to ensure only correct choice is provided in the Sex field. All correct formats of input are shown alongside the fields.
The message for discount is highlighted in red to catch the users attention. The fields marked as it are necessary to fill.
Buttons for cancelling the form, asking for help, saving the form, and going to the next screen is present on the form.
Products by Demographics Summary Report
As per the requirement of marketing department, the products by demographics summary report should give details about sales by regions, by product categories and customer categories. A sample Products by demographics summary report is as given below

Customer Purchasing Frequency report gives details about frequency of purchasing of customers, from which various conclusions about their purchases can be made.

This report is a personalized report which implies that upon asking this report about a particular customer by his/her customer id system generates this report. Description of this report is as follows
The report contains company name and date.
It shows the customer details, like Customer Name. Customer id etc., for whom this report has been generated.
The purchasing frequency is shown by a graph. The number of purchases made throughout the years is shown here, same can be generated for months also.
