Introduction to Principles of Personalised Care

Personalized care and support planning is the active participation of the management for the betterment and well-being of an individual. The process involves the strengths and weaknesses recognition as well as things and experiences that matter the health. They are used to address the actions and outcomes to resolve the matters. The social care services and health is received by personalized care and support planning. This is an integral tool designed to ensure the health and social care services. They are provided with the help of joined-up plans that covers wellbeing needs and health of the community. It allows us to value and manage the experts and participants in management and well-being to ensure solutions and outcomes in the personal context to their whole life that leads to chances of improvement in supporting them. Here, in this essay, the story of Helen and Karl will be discussed to provide personalized care and support plans. The assessment of the story will include the decisions, conversations, and agreed outcomes that will make sense in providing care by family and professionals. The proportionate, coordinated, and flexible plan will be followed for the improvement of the situation, care, and health condition of a person. The plan will describe the person, matters to him, and the necessary elements that will make the plan achievable and effective for the patient' s betterment (Rooij et al., 2018). 

Person’s Episode of Care

Amongst the several examples of care, Helen and Karl's story will be discussed to provide a personalized plan of care and supportive treatment. Karl aged 23 has suffered in this story from Spina Bifida and Hydrocephalus. It has resulted in damage to his nervous system, moreover, he suffered from causes and brain seizures. He has been suffering from complex ailments and needs a complex set of care. He was having access to Personal Health Budget (PHB), happier and with a feeling of control. When Karl shifted from her childhood to adulthood, his mother observed that he was having a change in his emotional state. He felt that he has no place in society because he was too stressed and disturbed by his mental illness. He felt anxious and depressed and made the activities for him very difficult to manage.

In this way, his mother, Helen gave more time to his son and bringing the tips for him to get recover. This all happened because he was given Personal Health Budget and he was able to manage all his assistance needs. His family supported and provides him personalized care to get recover from a serious illness.

Importance of Partnership Between Nurse and Patient

The partnership between nurse and patient is always fulfilling and rewarding. However, it includes the provision of physical and emotional support to the patient. They provide the services in patient satisfaction by maintaining a courteous and interpersonal relationship for the maintenance of the overall health and well-being of an individual. The establishment of a partnership between nurse and patient is vital in healthcare. The nurses treat a patient with professionalism, maintenance of eye contact, and with a display of confidence. They explain the plan of care for review and effective decision-making. It helps in ensuring the maintenance of patient privacy to provide compassion and care in treatment. The nurses are made aware to ensure personal boundaries for sensitive care provided to the patient. The partnership with the patient ensures that he/ she is provided with medical information and handouts in primary language that an individual understands (Beck et al., 2018). These items provide openness of communication and trust within both parties. Nurses are the frontline members that are involved in providing patient care from the first point of contact.

They are involved in building trust and relationships for the improvement of the patient. In the case of the patient like Karl, they are admitted to Daycare centers where the partnership between nurse and patient has developed a therapeutic relationship. This has changed the scenario of bed occupancy rate and length of stay. The nurses are engaged in many ways to get engaged in patient care and provide the best possible care. The mindful approach is followed in experiencing distress and behavior vitality for handling intense patients with psychological disturbances. This approach is used to provide concentration on thought processes like 'frontline', 'common ground' and 'shift'. All these three in nursing care helps in visualizing movement in space and time. This theme of a mindful approach accounts for nurses and patients to relieve them from distress and prevent rising conflicts. They get a place for courage and confrontation of disputes in handling mentally ill patients. Here, the nursing staff in this department responds to patient needs very quickly. It has become the common ground for both patients and nurses to talk about the concern and seek advice for clarity. It is the presence of knowing and respectful person to person connection that enables the commitment to a shared base and understanding. This helps the patient in understanding that frontline managers encounter all the defensiveness and tension in intersubjective space. Hence, both the nurse and patient have a relational shift for interaction (Needham et al., 2018).

Use of Communication and Interpersonal Skills

Communication is the fundamental skill followed in a clinical profession, to perform the duties effectively and efficiently. It facilitates building trust and the relationship between medical staff and patients with a truly therapeutic alliance. In addition to this, the facilities and competence of professionals manifest kindness, openness, and attention in providing care to patients. The way medical personnel responds to the patient's illness is the element that boosts performance, contributes to the growing interest and prestige of medical professionals. It is the means that is followed as the purpose of promotion, placement, selling, and pricing medical services. In general, it is the means that is followed to process the information, follow the interaction and involvement of the receiver and sender. The communication provides data that is relevant for diagnosis and undoubtedly builds a nurse-patient relationship. The combination of verbal and non-verbal communication accompanies the attributes of communication. Informed consent in an interpersonal relationship has great importance in the healthcare system.

Paraveral forms and non-verbal forms are important for the perspective of capital formation, emotional support, and sympathy that exists between two parties to share logical and semantic values. The continuous change of information leads to the fulfillment of the objectives over the state of disease and evaluation. Hence, it leads to the contribution of better performance and getting effective results as patient outcomes. 

Advantages to Interprofessional Teamwork

These days, healthcare is delivered with a team-based approach. They work on the fundamental of working together and delivering quality care to the patients. It is because the problems have arisen in the healthcare sector and it needs collaboration of teams with the priority to serve the patient with the best possible care. It involves data sharing and efficient communication to engage the members with patients for the goodwill of the patient. It follows the active participation and engagement of members with leadership and support to fulfill patients' needs to foster respect with interdisciplinary collaboration. Interprofessional collaboration in healthcare involves the use of technology and strategies in support of the team. The patient care outcomes improved with the interprofessional team care management. Also, the reduction is there in medical errors, treatment fastening, reduction in inefficiencies and healthcare costs, and improvement in staff relationships and job satisfaction. Effective team care management is one of the benefits of an advantage to interprofessional teamwork to get positive results in patient care management. The need has been analyzed due to an increase in morbidity and mortality rates in the complexity of care patterns (Barker et al., 2018).

The evolution in the healthcare system is demanding quality care to patients with a great focus on the patient care approach. This is only achieved if a wide range of principles and values are shared within the team members. They help information and development of an effective team to deliver exceptionable care to patients. It aims towards following strategies, policies, and practical experiences to motivate the employees to achieve the goals of the organization and overcome challenges. It is followed globally by all the healthcare units to construct a patient-centric approach and effective care management. They work toward common vision, mission, goal, and objective with the set of two or more people. It is the approach of healthcare services that share accountability and responsibility to prevent the patients from immediate risks. The adverse events like medication errors and near misses are controlled with the support of a coherent team and identification of errors to control them. The team members are merged based on expertise, communication, decision-making, and observations to optimize patient care. The culture of the workplace and team efforts make the team perform well. \

These days, a team of healthcare professionals is involved in taking care of the patient. They establish effective teamwork for providing safety to the patient and minimizing the adverse events that cause misunderstandings of responsibilities and roles. Moreover, patients are also involved in their care to minimize the chances of error and adverse events. The new technological means are followed by the change in healthcare systems. It is not the work of single healthcare professional to handle a patient in all means. To get more benefited, the healthcare teams are specialized with skills and knowledge and other functions to bear workload in certain situations, They know about their roles and responsibilities in a team and interact with each other for providing care to the patient.

They possess knowledge and skills with specialized care and take relevant decisions. Values, beliefs, and ethics play a major role in providing effective care in the healthcare profession. They deal with honesty, discipline, humility, creativity, and curiosity. Many models are followed by them such as shared goals, clear goals, effective communication, leadership, and measurable processes and outcomes. It results in an improvement in the healthcare team at all levels and reaching the patient with the best possible ways to treat him. This will help in enhanced job satisfaction, enhanced well-being, greater role clarity, improved health outcomes, reduction in medical errors, client satisfaction, acceptance of treatment, accessibility of services, reduced time in hospitalization, and many more (England, 2018).

The Barrier to Interprofessional Teamwork

Interprofessional culture is sometimes harmful in healthcare settings. It is the same as 'the social heritage of a community, the total of the professions, ways of thinking and behavior that distinguishes one group of people from another and which tend to be passed down from generation to generation'. Everyone follows different cultures, beliefs, and values in a setting. They often get the opportunity to constitute and collaborate within the barriers. As an example, the group of healthcare is not involved with full team participation in the advocacy of patients due to some reasons. Everyone like occupational therapists, nursing personnel, doctor, physiotherapist, and massage therapist have different ways to handle the patient with the same illness. This is known that physicians have the major role to play in healthcare services but this hierarchy is broken among the professionals to the values that they take into account. They outstand with their professional identity and are rigid with their attitude to change with time. They even do not address the students in their professional training. Hence, they are said that physicians who practice in this way have very little scope to learn according to the changing trends and demonstrate their knowledge, attitudes, and beliefs within the team. Also, they use different terminology in their processes during the implementation of care. The nursing members are, however, not involved by them to get influenced while decision-making. Hence, this results in different styles of clinical interaction, not a full collaboration of nursing members and staff in patient care, conflict resolution, negotiation, and humor (Wilson, Woollands & Barrett, 2018).

Role of Family and Carers with The Means of Social Prescribing in The Improvement of A Person's Health and Well-Being

Personalized care is provided to patients using social prescribing. It develops organizations and people for leading the local means in healthcare systems. Social prescribing is used in experiencing health outcomes of people, carers, and families through the system of values in healthcare. Thi way the local agencies link to a worker and focusing on what matters to them to achieve health and well-being. They connect people to community groups to get sustainable and accessible help to work collaboratively with partners. A wide range of people is involved in social prescribing that includes long-term conditions to people who need support according to their mental illness. These are those people who are kept in isolation and deserve complex social needs that affect well-being. The wide range of local agencies is linked such as caregivers, voluntary associations, social care services, allied healthcare professionals, and housing associations. The model is developed with the partnerships within the stakeholders that work on the easy referral of local agencies, a framework for common outcomes, workforce development, support for community groups, matters that create personalized plans, and partnership and collaborative commissioning at work. In this, the approach that is followed is active signposting.

This approach is a light-touch approach that includes the use of resources and knowledge. It works best for the people who are skilled and confident to find their ways after little intervention. It benefits a large number of people by providing round the clock personalized care. Social prescribing is the link between multi-disciplinary members to serve in many networks. This intervention is biggest on its own and involves a community-based approach by the national health system and supporting treatment plans in personalized care. Many people, systems, and communities are actively involved in social prescribing to began the contract with a multi-disciplinary approach. A large number of link workers are involved in this program. It has been seen that social prescribing has a positive impact on the well-being and health of people for improvement in quality life and emotional well-being. This mechanism has reduced workload on general practitioners and enhanced the need for social prescribing in the healthcare system (Brown, 2018). 

Conclusion on Principles of Personalised Care

From this essay, it has been understood that personalized care and support planning is the systematic process that is based on better conversations between health and social care practitioners. The overall aim of providing a good life and support can be achieved by this mechanism. It ensures support and coordination among the team members in bringing together with experience and technical expertise to find out the solutions. This practice is carried out by voluntary and statutory service providers to ensure they are taking effective decisions. The outcome achieved is the single plan, no matter the condition and issues in the process. The emphasis is laid on following the continuous process and not the whole event. The care is therefore achieved by introducing the activities within the community and coordinating with social care at the time of need. This approach is applied in both the immediate and long-term well being of a person. In the cases of mental illness like Karl, the care program approach is followed to endure the services into mental health issues with long-term care and support according to the wish of the patient. This helps in enhancing support planning and personalized care throughout the system. Overall, this is an integrated approach that brings all the resources together for functioning and benefiting the individual.
