Transforming health care increases the need to utile the person-centred approach during the care to improve the quality of life of the patient. The new health care systems have more emphasis on health promotion and community care to reduce the chances of health-related issue majorly encountered in the community.
Nurses are considered to play a major role to implement the transformation accurately by utilizing positive professional practice during the care. The nurses are expected to utilize the strength-based leadership skill to improve the collaborative partnership with team members and patient to improve the quality of care. The strength-based approach utilizes three aspects during the care process to improve the effectiveness of the care which include recognition, mobility and develop capabilities. The health promotion conducted at community level utilizes the collaborative effort of the health care professional to reduce the chances of health care issue related to disorders. The partnership is considered to be the multidisciplinary approach that utilizes collaboration of the different organization in the health promotion at the community level that helps to improve the individual health in the community. There are certain advantages to utilize the collaborative method in health promotion at the community that include improved lifestyle, better health, sense of partnership and decrease health compromise behaviour. The partnership during community health promotion is considered to improve the quality and sustainability of the health-related intervention by reducing the chances of issue arise due to disorders (Cicognani et al., 2019).

Many interventions require community-based health promotion approach due to their increased prevalence and less understanding of the individual’s one of the disorder include cardiovascular complication. The objective of the community-based health promotion intervention is to improve the understanding of the major group of the society regarding the issue so that it can be possible easily accessible to every section of the society. The community health promotion program utilizes the partnership approach with the individual in the community to improve their understanding regarding the issue and possible precautions (McNeish et al., 2019). Nurses can utilize their clinical skill to assist the patient to improve their understanding regarding different risk factor that can lead to cardiovascular disorder.
Nurse and patient coordination can help to improve the patient understanding regarding necessary precautions that are required to decrease the chances of issue related to the cardiovascular system. The best method that can be applied to improve the understanding regarding dietary component followed by physical exercise that contributes to better health status (Jorstad et al., 2015).
